Retail Store Drawing: Official Rules
Enter to win a $200.00 gift card at any participating Retail Store of an Affirmative Insurance Holdings, Inc. ("Sponsor") subsidiary, including A-Affordable Insurance Agency, Inc., InsureOne Independent Insurance Agency, LLC, Yellow Key Insurance Agency, Inc., Driver’s Choice Insurance Services, LLC, and Fed USA Retail, Inc. Contest begins November 3, 2008. Drawings will be held once each month. Winners will be selected at random from all eligible entries received during the prior calendar month. Odds of winning depend on the number of eligible entries received. First drawing will be held in December 2008. Winners will be notified within 30 days of drawing.

Purchasing will not increase your odds of winning. Must be eighteen (18) years or older to enter. Entries must be submitted in person at a participating Retail Store. Limit one (1) entry per person per month. Persons submitting more than one (1) entry per month will be deemed ineligible. Employees of Sponsor or any of its affiliated companies, their immediate families (spouses, parents, children, sibling or their respective spouses), and those living in the same household are ineligible. Winners will be notified by telephone or electronic mail. Winner must pick up prize in person at a Retail Store. Winner will be required to sign a Release and Consent and allow Sponsor to take and use their picture for advertising purposes as a condition of receiving the prize. Any federal, state and local taxes are winners’ sole responsibility. No transfer, assignment, cash redemption or substitution of prizes except by Sponsor due to prize unavailability and then for a prize of equal or greater value. Certain other restrictions may apply. El concurso comienza el 3 de noviembre de 2008. Los sorteos se realizarán una vez por mes.
